What is Pelvic Organ Prolapse?

Pelvic Organ Prolapse (POP) is a structural condition that occurs when the muscles, ligaments, and connective tissues within the pelvic floor stretch, weaken, or tear. The pelvic floor acts as a supportive hammock, keeping your pelvic organs—including your bladder, uterus, cervix, and rectum—securely in place. When this supportive network loses its strength, one or more of these organs can slip out of their normal positions and bulge downward into the vaginal canal.

Depending on which organ is shifting, prolapse is categorized into distinct types:

  • Cystocele: The bladder drops and bulges into the front wall of the vagina.
  • Uterocele (Uterine Prolapse): The uterus and cervix slip down into the vaginal canal.
  • Rectocele: The rectum bulges into the back wall of the vagina.
  • Enterocele: The small intestine pushes into the top of the vaginal wall (most common after a prior hysterectomy).

What are the most common symptoms of pelvic organ prolapse?

Many women with mild prolapse experience no symptoms at all. However, as the structural displacement progresses, you may notice:

  • A persistent sensation of heaviness, pulling, or fullness in your lower abdomen or pelvis.
  • The feeling that something is physically sitting or slipping out of your vagina.
  • A visible or palpable bulge or lump inside or protruding from the vaginal opening.
  • Lower back pain or a dull pelvic ache that worsens after standing for long periods or lifting heavy objects.
  • Difficulty emptying your bladder completely, a slow urinary stream, or recurrent urinary tract infections (UTIs).
  • Chronic constipation or the need to physically press against the vaginal wall to help pass a bowel movement (splinting).

What causes the pelvic floor to weaken enough to cause prolapse?

Pelvic organ prolapse is generally caused by long-term increased pressure on the abdomen and pelvic floor. Common risk factors include:

  • Pregnancy and Childbirth: Particularly long, difficult labors, pushing for an extended time, or delivering large babies vaginally.
  • Aging and Menopause: The natural decline in estrogen during menopause reduces the elasticity and strength of connective pelvic tissues.
  • Chronic Straining: Frequent straining due to chronic constipation or a persistent, heavy cough (such as from smoking, asthma, or bronchitis).
  • Repetitive Heavy Lifting: Job-related lifting or high-impact, heavy weight-training.
  • Obesity: Carrying extra body weight places continuous, daily pressure on the pelvic floor muscles.
  • Prior Pelvic Surgery: Such as a hysterectomy, which can alter the natural support system of the remaining organs.

Can pelvic organ prolapse heal on its own?

Stretched or torn pelvic ligaments and tissues cannot fully repair or tighten back up on their own. However, mild to moderate cases of prolapse can be successfully managed, stabilized, and often improved without surgery.
